Objective

To ensure acquisition of in-depth theoretical and practical skills on “Health of Mother and Child. Gynaecology and Obstetrics”, in accordance with the competence requirements of the family doctor specialty, prepare a specialist who is able to provide constant, uninterrupted and comprehensive care of the patient and their family members and help the patient make a choice between the offered health care services, ensuring the patient’s abilities for better physical, mental and social functioning, is able to assess the patient’s state of health, diagnose diseases and provide supportive treatment according to the defined competences. To prepare the doctor for certification in the specialty of family doctor, in accordance with the regulatory documents of the Republic of Latvia.

Learning outcomes

Knowledge

1.In women’s health care, the medical resident:
1. Has knowledge and understanding of normal female anatomy and physiology, stages of reproductive development:
- performing gynaecological examination,
- menstruation cycle (physiology, menstrual cycle disorders, premenstrual syndrome),
- menopause (normal physiology, hormone replacement therapy, emotional and sexual problems),
2. Has knowledge and understanding of care and examination of pregnant women,
- in the care of a pregnant woman, be ready to manage a normal pregnancy, to manage the course of a normal birth, in the evaluation of embryonic development, in the evaluation of pregnancy-related diseases, in emotional and sexual problems,
- in obstetrics, family counselling on childbirth, postpartum care,
3. Has knowledge and understanding of the main threats to a woman’s life and health:
- abortions (spontaneous, artificial; indications, complications),
- childbirth,
- injuries,
- malignant tumours,
- cardiovascular and other diseases.
4. Orients oneself in the specifics of preventive work, rehabilitation measures:
- contraception,
- preventive measures (cancer screening programmes, oncocytology, breast examination, mammography, gynaecological examination, occult blood test, prostate cancer screening),
- osteoporosis prevention (exercise, physical load).
5. Has knowledge and understanding of specific women’s health problems:
- inflammations in the small pelvis,
- endometriosis,
- precancerous conditions,
- tumours,
- chronic pelvic pain,
- vaginitis,
- sexually transmitted diseases,
- sterility, problems related to pelvic muscle dysfunction: urinary incontinence, uterine prolapse,
- psychological problems: domestic violence, rape, postpartum, post-abortion depression.

Skills

1.Skills to be strengthened during the family medicine education programme – examination, treatment methods, disease prevention and health promotion measures:
Gynaecological procedures:
- gynaecological examination,
- breast examination, palpation,
- taking bacteriological and cytological smears from the vagina, cervix,
- explanation of hormonal contraception issues,
- insertion and removal of intrauterine coils,
- advice on cervical ring placement,
- determination of the time of pregnancy,
- assisting in childbirth,
Skills in women’s care:
- consulting on issues related to birth control,
- insertion and removal of intrauterine spirals, insertion and removal of the cervical ring,
- bladder catheterisation,
- collection of the oncocytologic sample,
- breast palpation,
- consulting a pregnant woman,
- diagnosis and management of normal pregnancy,
- care in the postnatal period.
Health examination and related consultations:
- knowledge of diagnostic methods,
- assessment of physical work capacity,
- explaining the harmfulness of tobacco,
- sexual education,
- counselling in cases of psychological problems, stress management,
- education about hormone replacement therapy for menopausal women.

Competence

1.The acquired competences allow to justify decisions, to perform additional analysis if necessary:
- Able to explain the impact of environmental changes on the incidence of diseases in the population, to understand the application of preventive, diagnostic and therapeutic methods in this aspect.
- Able to assess patient complaints, their connection to the patient’s state of health. Has a broad understanding of diseases and risk factors thereof, the most common diseases in the population, care in case of chronic diseases, diseases that cause disability, and conditions that require emergency care.
- Has an understanding of the use of preventive and diagnostic manipulations in primary health care.
- Ability to provide treatment to the patient at the level of primary care, organise rehabilitation if necessary.
- Understanding of diagnostic possibilities at the primary and secondary level of health care, the possibilities of referring the patient to a specialist doctor if necessary.
- Ability to organise and conduct consultations on issues related to the organisation of health care, especially in matters related to maternal and child health care, family planning.
