About Study Course
Objective
To ensure the acquisition and development of theoretical knowledge and practical skills in accordance with the Internist specialty regulations adopted in Latvia and the European Union requirements for this specialty, which would allow the doctor to orientate himself in emergency medical assistance by reading literature or encountering in clinical practice, as well as allow him to provide professional tertiary healthcare level assistance to the patient, his family and colleagues.
Learning outcomes
1.During the time allotted for the study course, the resident acquires knowledge about emergency medical assistance, diagnostic algorithms and principles of treatment within its framework.
1.As a result of the study course, the resident will be able to evaluate the patient's clinical condition, collect a full life and disease anamnesis, objectively examine the patient, if necessary prescribe additional examination methods, prescribe appropriate treatment, as well as provide emergency medical assistance in acute situations.
1.• recognize emergency situations in medicine;
• to create an individual plan for examination and treatment of the patient;
• choose specific diagnostic methods, evaluate their level of information, time of application.
The following competences are essential, but not exhaustive (the spectrum of competences can be expanded if prerequisites arise during studies):
• Advise patients and family members;
• collect anamnesis;
• perform a physical examination;
• assess nutrition;
• perform a clinical evaluation and make a decision;
• provide explanations and consultations;
• encourage and provide professional support.
Assess the clinical manifestations, order an examination, establish a diagnosis and agree on a treatment plan:
• know and assess the severity of clinical manifestations;
• perform differential diagnosis;
• order the necessary investigation and interpret its results;
• consult with specialists from other branches;
• agree an appropriate management plan for the health problem with patients and carers.
Immediately provide care to the patient in emergency situations, including first aid and resuscitation measures:
• know and assess acute emergency situations;
• provide emergency assistance;
• provide cardiopulmonary resuscitation to patients according to current European guidelines.
Prescribe nutrition, drugs, fluids and blood products:
• determine clearly and precisely nutrition, medicines, fluids and blood products;
• coordinate nutrition, medications, fluids and blood products in the clinical context;
• review the appropriateness of treatment and evaluate potential benefits and risks;
• treat pain and suffering.
Take practical action.
The internist is competent in many practical manipulations (see list of manipulations).
For any of these manipulations, the ability to:
• provide relevant information to the patient;
• obtain informed consent, if necessary;
• use as minimally invasive procedures as possible;
• reduce pain and suffering;
• use appropriate hygiene and infection control measures;
• make appropriate records of manipulations in the medical documentation.
Communicate effectively with the patient in the context of the illness:
• communicate with patients;
• communicate with the caregiver and relatives;
• communicate with specialists from other industries;
• ensure continuity of care;
• stop or mitigate bad news, skillfully explain it;
• communicate with disabled patients;
• communicate with those who need the help of an interpreter;
• seek and receive informed consent where necessary;
• communicate in writing, including medical documentation, telephone and other means of communication;
• act appropriately in cases where aggression is observed.
Apply ethical and legal principles in internist practice:
• apply ethical principles and analysis in clinical care and research;
• apply national and European Union legislation in clinical care and research;
• assess the patient's ability to give consent;
• obtain and record informed consent;
• provide information on organ donation;
• certify death;
• prepare a request for an autopsy.
Assess the psychological and social aspects of the patient's illness:
• evaluate the influence of psychological factors on disease manifestations and disease;
• evaluate the influence of social factors on disease manifestations and disease;
• assess the impact of the patient's condition on work;
• detect alcohol and drug use and addiction;
• cooperate in a timely manner with relevant colleagues.
Apply evidence-based medical knowledge, principles, skills:
• use evidence in practice;
• identify and conduct an appropriate literature search;
• critically evaluate published medical literature.
Effective use of information and information technologies in the context of infectious diseases:
• prepare accurate, timely and complete clinical documentation;
• be able to access international scientific information;

• demonstrate an understanding of research design and methodology, including statistical analysis.
To promote the health of the individual and the population, to operate effectively in the health care system:
• comply with the screening, monitoring and immunization programs established by the state;
• apply measures to prevent the spread of infection;
• evaluate nutrition and provide useful advice and support in matters of healthy nutrition, including being able to manage cases of obesity and malnutrition;
• promote patient safety and accident prevention;
• ensure compliance with professional certification practices;
• demonstrate an understanding of the structure, organization and operation of the health care system in which to work.
