Treatment of Spinal Fractures and Their Consequences (RMK_015)
About Study Course
Objective
To ensure the acquisition and improvement of theoretical knowledge and practical skills in the diagnosis and operative treatment of spinal injuries with various methods and approaches.
To prepare a medical specialist for the recognition of an acute and urgent condition, to systematize the findings of examinations and the clinical picture, which would clarify the diagnosis in a short time. To build the ability to make decisions and, if necessary, perform operative treatment urgently within the first hours. The aim of the course is to combine the previously discussed knowledge and practical skills in order to provide the specialist with appropriate quick and targeted action, the ability to involve other specialists and justify the decisions made.
Knowledge of the most common surgical techniques for emergency spine operations.
Practical training to learn high-quality anterior approaches to the spine in the thoracic and lumbar regions - thoracotomy, lumbotomy and laparotomy methods. Systematize knowledge of complications in these surgical techniques.
Learning outcomes
1.Theoretical knowledge covers the typical acute conditions in the practice of a spine surgeon, the completion of the course provides the ability to work in inpatient facilities that provide round-the-clock multi-profile medical assistance or in a specialized medical center.
• Detailed theoretical knowledge of the biomechanical classification of spinal injuries, typical complaints, objective symptoms and finding of radiological examinations, their purposeful appointment, appointment of additional examinations.
Drawing up a treatment plan depending on the type of vertebral fracture and ligament damage. Systematized and justified decision-making about acute or planned surgery, one or more stages of spinal stabilization, choice of implants.
Patient preparation for acute operative care, including patients with co-morbidities and antiplatelet, anticoagulant users, patients with polytrauma. Informing the patient and relatives and preparing medical documentation for acute spine surgery. Positioning with or without fixation in a Mayfield clamp.
Indications for use, application and adjustment technique of Halo external fixator.
Neck repositioning, decompression and stabilization operations with ventral approaches. Reposition technique of dislocations of the cervical vertebrae. Open reposition and closed. Patient preparation for thoracic vertebra fracture surgery. Positioning, closed reposition. Transpedicular screw fixation technique for thoracic and lumbar vertebral fractures, open reduction using implant construction. The technique of decompression of the spinal canal and closure of lesions of the spinal cord in the thoracic and lumbar spine. Anterior approaches to the thoracic and lumbar spine.
Postoperative care and drug therapy for an uncomplicated patient of the above operations. Postoperative imaging, its evaluation. Appointments for nursing staff and physiotherapist. Preparation of check-out documents. Early recognition of complications in the postoperative period, treatment of complications.
Emergencies in spine surgery are often associated with acute gross neurological deficits. Within the framework of this course, students acquire systematized knowledge of more frequent acute conditions and the actions of a spine surgeon.
1.Sufficient to perform emergency operations in spine surgery and to perform spine stabilizing and nerve structure freeing operations in trauma cases.
1.The student is competent to determine the indications for urgent spine surgery or, on the contrary, to make a reasoned decision that operative treatment should not be performed.
